Apply for a MOFAS Chemical Health & Recovery Grant

On behalf of the Minnesota Department of Human Services (DHS), the Minnesota Organization on Fetal Alcohol Syndrome (MOFAS) is seeking applicants to provide comprehensive, gender-specific services to pregnant and parenting women known to use or abuse alcohol or other drugs in order to increase the number of toxic-free babies born.

Important Dates

Pre-proposal Conference In Person October 16, 11:00 am – 12:30 pm
Pre-proposal Conference Webinar October 17, 10:30 am – 11:30 am
Deadline for Submitting Questions October 17, 4:00 pm
Grant Application Due November 6, 2017, by 4:00 pm

The grant period will begin as soon as the grant review and selection process is completed and will end on June 30, 2019.


Application Process

Step 1:  Review Request for Proposal

Review the Request for Proposal (PDF) to determine if this grant-funded project is appropriate for your organization.

Step 2:  Review FAQs

Review these Frequently Asked Questions (PDF) about the application requirements to ensure that you meet eligibility criteria.

Step 3:  Review Grant Contract Terms

Review the Grant Contract Terms before submitting the grant application.  In the application, applicants will signify their acceptance or attach exceptions to the contract terms.

Step 4:  Participate in Preproposal Meetings

You are encouraged, but not required to participate in a Preproposal Meeting. If you are unable to participate and have questions, please submit those questions in writing by 4 pm on Tuesday, October 17, 2017, to Kendra Gludt, kendra@mofas.org.

Step 5: Follow Up Responses

Responses to submitted questions will be posted here.

Step 6:  Submit a full grant application

Applications are due on Monday, November 6, 2017, by 4:00 p.m.

Step 7:  Review of grant applications

A review process will be conducted by MOFAS and will include site visits and a review committee.

Step 8:  Notification of recipients

Organizations that are approved to receive grant funding will be contacted by MOFAS staff. A grant agreement will be developed that details project dates, payment schedules, required grant reports, and other pertinent information. Organizations will be notified via letter if their project is not selected to receive funding.
